At The Hereditary Cancer Foundation, we know the importance of getting tested and KNOWING YOUR RISK of a hereditary cancer mutation. Our organization does not want costs to be a barrier for people getting tested if they meet criteria.

If you have been tested and need assistance in paying for the cost of your testing, please follow the following instructions to see if you qualify.

Step 1: Apply

Here are some important notes to consider before applying:

  • This form must be completed in its entirety to be considered.
  • This form must be completed by the PATIENT.
  • We may need to obtain additional information from the Testing Provider to proceed. 
  • This form must be completed PRIOR to or the initial stage of getting tested. 
  • Patients CAN NOT get personally reimbursed for testing costs. 
  • Payments for testing will be sent DIRECTLY to the testing lab. 
  • You may be asked for copies of your bill from the testing lab.
  • You may be asked to complete Financial Assistance through the testing lab.

Step 2: Determination

Once you have COMPLETED this form, our organization will make a determination as soon as possible. You will be contacted with a determination or the need for further information within 14 days. If you have questions in the meantime, please reach out to us via email at financial@hereditarycancer.org 

ADDITIONAL DOCUMENTS THAT MAY BE NEEDED TO DETERMINE FUNDING

    • Signed Release of Information
    • Bill from Testing Lab
    • Financial Assistance Determination from Testing Lab
    • Insurance coverage
    • Insurance denial documents, if it has been denied/appealed

 

STEP 3: RESULTS AND FOLLOW UP

If The Hereditary Cancer Foundation has funded your testing, we ask that you consider letting us know your results.

Our organization relies on donors and grants to continue helping patients pay for their hereditary cancer testing. Sharing your results will allow us to compile un-identifying personal data to show the impact funding testing is making.

The Hereditary Cancer Foundation WILL NEVER SELL OR SHARE YOUR personal information.

If you are approved Financial Assistance, your balance will be paid DIRECTLY to the testing lab.
